OTHER QUALIFICATIONS
If you do not have a professional certification listed above, include the following information as an attachment to this application:
  • Proof of current employment or active regular work in the hearth industry performing installations, maintenance, and cleaning activities on wood burning heaters as defined in Section 3.0 of District Rule 4901
  • Proof of three years of work experience that qualifies you to perform installations, maintenance, and cleaning activities on wood burning heaters as defined in Section 3.0 of District Rule 4901
  • Supporting documentation demonstrating three years of hearth industry experience referred above in installing, maintaining or cleaning these types of devices such as but not limited to:
    • Your business license
    • Any manufacturer certifications received
    • A contractor's license indicating this type of expertise
    • Copies of past invoices that detail your work performed on these types of devices
    • Other certifications or accreditations that would indicate knowledge or experience in installing, maintaining and cleaning these devices
  • Up to three professional references verifying experience, qualifications and relevant work experience

REGISTRATION TERMS
  • Submittal of an application to become a Registered Wood Burning Heater Professional (RWBHP) does not guarantee application approval. Completion of the San Joaquin Valley Air Pollution Control District (District) provided training workshop and a fully executed contract signed by all parties are required to consider and promote yourself as a District RWBHP.
  • Registered wood burning heater professionals are not employees or agents of the District.
  • Registration as a wood burning heater professional is not an authorization to act on behalf of the District. Providing inspections, maintenance, and cleaning activities on wood burning heaters as defined in Section 3.0 of District Rule 4901 is not an action on behalf of the District.
  • To the fullest extent permitted by law, the wood burning heater professional agrees to release, waive, discharge and covenant not to sue the District for liability arising out of the services performed as a wood burning heater professional.
  • Registration with the District is a verification that the wood burning heater professional is qualified to perform inspections required by the District per Section 5.7.3.1.2 and Section 5.8.2.1 of District Rule 4901 (Wood Burning Fireplaces and Wood Burning Heaters).
  • The wood burning heater professional registration is only valid for a three (3) year period and must be renewed if continued participation is desired.

I understand that my registration as a Wood Burning Heater Professional may be revoked if at any time the San Joaquin Valley Air Pollution Control District discovers that any of the information I provided is untrue, inaccurate, not current, or incomplete.
